SEVIS Update Form	Form to submit a notice of change of address, name, major, or financial status---REQIRED SEVIS INFORMATION
Extend Program of Study Form	If you need to extend your I-20, this form must be completed by your academic advisor before meeting with the International Student Advisor.
Reduced Courseload Form	Academic or medical reasons for taking less than required minimum number of credits
Curricular Practical Training Form	(CPT)Required authorization to accept paid internships
Optional Practical Training Form	(OPT)Application processs to apply for employment related to student's major area of study.
Transfer In Form	Form used to transfe to NAU from another university or college in the U.S.
Transfer Out Form	Form used to transfer from NAU into another university or college in the U.S.
Change Status to F-1	Procedures to change to F-1 status from another status
Reinstatement and Return to F1 status	Information and instructions to regain F-1 status through either travel out of and return to the US or by applying to the US Department of Homeland Security
J-2 Work Authorization Form	Instructions to apply for J-2 work authorization from US Homeland Security
F1 Employer Form	Form to be completed by an F-1 student's employer-this is required for the application for a US Social Security Number

Academic Training (J-1):

Academic Training (AT) : Work, job training, internship or experience related to a J-1 student's field of study.
